Progress Reports & LPCC Update

Hello Everyone!

As we approach 8/1, we want to remind you of the new Provider Progress Report Distribution Policy:

To align with state policy, providers are responsible for emailing reports to ALL members of the IFSP team. This includes the Lead ED Team Rep. You can find the Lead ED Team Rep listed on Section 8 of the IFSP.

Providers may send to all team members in the same email. For example, if a child receives DT, OT, and PT services: the DT emails his/her report to the SC, OT, PT, and Lead EDT; the OT emails his/her report to the SC, PT, DT, and Lead EDT; the PT emails his/her report to the SC, OT, DT, and Lead EDT.

As always, the provider is responsible for reviewing and providing a copy of his/her report with the family. Also, the SC submits all received reports to the SPOE for the EI record.
